Skilled Nursing Care

Skilled nursing care offers continued treatment to those patients who may need additional rehabilitation following a hospital stay. The goal of skilled nursing care is to restore the patient back to their highest level of independence and functioning as quickly as possible. With skilled nursing care, you are under the observation of a physician who is responsible for an overall care plan and supervises your progress.

CHI St. Joseph Health has the largest skilled nursing care network in the region, with multiple locations. Our comprehensive team of physicians, nurses, social works and chaplains work together to create the best treatment plan unique to each patient.

Making a decision about where to access care for a family member can be stressful. Our team is committed to giving you the best experience and getting you back to your life, with the help of your family and caregivers. Skilled nursing care is a choice for you, your family and your physician. To facilitate the process at any CHI St. Joseph Health facility, work with your case manager and physician.
