Medicare ACO

As an Accountable Care Organization (ACO) working with Medicare, CHI St. Joseph Health Partners ACO is a partnership between CHI St. Joseph Health and ACO professionals. We are a group of doctors and other healthcare providers who agree to work together with Medicare to give you the best possible care. ACO participants include CHI St. Joseph Health and many participating providers and clinics.

Our goal is to provide you with the best possible, comprehensive care, by making sure that all of your providers have the most up-to-date information about your health and your care. For you, this means your doctors communicate better with each other, and you avoid having duplicate tests, or answering the same questions over and over.

CHI St. Joseph Health and CHI St. Joseph Health Partners came together to form the ACO. The ACO composition is made up of networks of individual practices of ACO professionals, hospital employing ACO professionals, Critical Access Hospital (CAH) billing under Method II, Rural Health Clinic (RHC).
